Please be sure to fill out an intake form in the studio to go with this online purchase. You can get one from any of the instructors at the front desk. Return the intake form directly to an instructor only, do NOT leave it on the desk.

By purchasing this membership, you attest that you have read, understand, and agree to abide by our Code of Conduct, our Sparring Protocol, and further understand that violating our policies may result in expulsion without reimbursement.

By purchasing this membership, you understand that it cannot be refunded in full or in part after purchase has been completed.

The membership purchase below is for 1 month and does not recur. Your month of membership counts from day of purchase. To have a monthly recurring membership, please refer to the intake form noted above.
